CHAPTER 1
Hello, my name is Professor Gina Forsythe, and I am a college lab technician here at Kingsborough Community College. Today I'll be showing you how I set up for an open pancreatectomy.
CHAPTER 2
I have already opened the back table, draped the Mayo stand and the ring stand as well. My instruments, I have already verified the sterility with my circulating nurse. We have checked for the external and internal indicators, and everything is good to go.
CHAPTER 3
First, I'm going to make my rolled towel. The first thing that I want to do is prepare the instruments to be able to start my initial count with a circulating nurse. When setting up my forceps, I separate atraumatic from traumatic.
CHAPTER 4
I'm going to take my light handle cover, suction, and Bovie tip and place it on the Mayo. These are the first things that will be set up for the case.
CHAPTER 5
I'm going to load my knife handles with the blades, always making sure to use a needle holder and not do it by hand. 10 blade would go on a number three handle as well as my 15 blade.
CHAPTER 6
Now I am ready to count with my circulating nurse. Counting in the sequence of softs, sharps, and then instruments, I'm going to start with my laparotomy pads, which have no rings. 1, 2, 3, 4, 5. Ray-Tecs also that come in 10, so that should always be your starting count. 1, 2, 3, 4, 5, 6, 7, 8, 9, 10. Marking pen, two pieces, 1, 2. Yankauer tip, 1. Moving on to sharps, sutures. I have 1, 2. Blades, 1, 2. Hypodermic needles, 1, 2. Bovie tip, 1. Scratch pad, 1. Now moving on to instruments. Mosquitoes, 1, 2, 3, 4. Curved Criles, 1, 2, 3. Straight Criles, 1, 2. Kellys, 1, 2, 3, 4. Large Kellys, 1, 2, 3. Allises, 1, 2. Babcocks, 1, 2. Kochers, 1, 2, 3. Right angles, 1, 2, 3. Tonsils, 1, 2. Large needle holder, 1. Small needle holders, 1, 2. One large curved Mayo, one Metzenbaum, one curved Mayo, one straight Mayo. Parker retractors, 1, 2. Bulldogs, 1, 2. Senns, 1, 2. Loops, 1, 2. Poole suction is two pieces, so 1, 2. Weitlaners, 1. Forceps, I have Adsons with teeth, 1, 2. Tissue forceps with teeth, 1, 2. Adson smooth, 1, 2. Tissue forceps, smooth, 1, 2, and DeBakeys, 1, 2. Richardsons, I have 1, 2, 3, 4. Rakes, 1, 2. Army-Navys, 1, 2. Double-ended Richardsons, 1, 2, 3, 4. Deavers, 1, 2, 3, 4. Malleables, 1, 2. Balfour, we count in pieces. You have one blade, 2, 3, 4, the two screws.
CHAPTER 7
On the field, I already have my medication, which has been verified with the nurse. I have 1% lidocaine with epi, and the expiration date is September of 2025. I also have my syringe labeled, making sure that it's labeled with the medication, the dosage, and the expiration date.
CHAPTER 8
I have the gowns and gloves that I'll be using for the surgical team, which I'll place here. Then I have the drapes that we would be using for the case in the order that they're going to be placed on the patient, and I'll put that on the Mayo stand. And now we're ready to gown and glove the surgical team and proceed with the case.
