CHAPTER 1

Hello, my name is Dana Donovan, and I'm an Adjunct Lecturer here at Kingsborough Community College. Today I will be showing you the setup for a breast biopsy procedure.

CHAPTER 2

Before I begin, I wanted to mention that my back table, Mayo stand, and basin have already been draped. The instruments that you see here on the field were verified with my circulating nurse that the internal and external chemical indicators have met the proper parameters to ensure sterility.

CHAPTER 3

We also have medication already on the field that was verified with my circulating nurse as well to make sure that the dosage, the medication name, and expiration date are all correct. It has also been properly labeled, both the syringe and the cup.

CHAPTER 4

I will begin by setting up my instrumentation. First, I will take a blue towel and make my instrument roll. Everyone has their own way of setting up their instruments and their roll. When setting up my forceps, I like to separate them by non-penetrating and penetrating. Non-penetrating on the left, and penetrating on the right. My knife handles have been isolated in a secure place. While setting up your instrumentation, you wanna make sure everything is visible for you and your circulating nurse to ensure that the count runs nice and smooth.

CHAPTER 5

My light handle cover will be placed on my Mayo stand, as well as my electrocautery or Bovie. My suction with my Yankauer tip. All necessary drapes for the procedure. And the gown and gloves for the surgical team will be placed here.

CHAPTER 6

Now we will begin our initial count. First, we start with softs, then we move to sharps, then we move to instrumentation. The softs we have here are Ray-Tecs and laparotomy pads. Ray-Tecs should always be in packages of 10, and laparotomy pads should always be in packages of five. Make sure the white paper around the sponges is not removed until you are ready to count with your circulating nurse. Ray-Tecs: one, two, three, four, five, six, seven, eight, nine, 10. Laparotomy pads with no rings: one, two, three, four, and five. We have a marking pen, which will be two pieces: one, two. And a Yankauer, which we will count as one. Now we will move on to sharps. Sutures, we have one, two. Always make sure that the needle is visible to your circulating nurse. Hypodermic needles: one, two. Blades: one, two. Bovie tip and scratch pad: one and one. Next, we will move on to instrumentation. Mosquitoes: one, two, three, four. Crile: one, two, three, four. Straight Crile: one, two. Kelly: one, two three, four. Large Kelly: one. Allis: one, two, three. Babcock: one, two, three. Kocher: one, two. Tonsil: one, two. Right angle: one, two. Sponge stick: one, two. Small needle holder: one, two. Large needle holder: one, two. Straight Mayo: one. Curved Mayo: one. Metzenbaum: one. Okay, forceps, pickups with teeth: one, two. Adson with teeth: one, two. Adson, smooth: one, two. Dressing forceps: one, two. Debakeys: one, two. Rakes: one, two. Green loop: one, two. Cushing Vein: one, two. Senn: one, two. Parker: one, two. Weitlaner: one. Bulldog: one, two. Right Army-Navy: one, two. Richardson: one, two. Deavers: one, two. And malleable: one.

CHAPTER 7

I'm going to load my blades. So we have a 10 blade on a number three handle. And a 15 blade on a number three handle as well. Always make sure you load your blades with a needle holder. Our initial count is complete, and now we are ready to gown and glove the surgical team.
