CHAPTER 1
Hello, my name is Joseph Ojeda. I'm a surgical technologist. I'm here today at Eastwick College, and today I'll be showing you how we will prep for a breast biopsy.
CHAPTER 2
I already have my Mayo stand, back table, and ring stand prepped. I double-prepped my mayo stand and have made a neutral zone. I have double-draped my back table and I have supplies on it. And I have my ring stand already draped with supplies on it as well.
CHAPTER 3
On my back table, I have the following supplies: lap sponges, Ray-Tecs, saline, skin stapler, Bovie with tip and scratch pad, Telfa, sutures. I've received medication onto my field from my circulator and I have verified the name, strength, and expiration date and labeled accordingly, as well as with my syringe. I have done the same thing with my saline and have labeled accordingly.
CHAPTER 4
With my ring stand, I have towels, gowns, and gloves for the surgeon and his assistant as well as patient drapes. I also have light covers, skin prep, suction tubing.
CHAPTER 5
Next, I'll be working with my circulator Ana to get my instrument tray onto my back table and to verify that there is no contamination in the tray. No moisture, no strikethrough. No moisture, no strikethrough. No moisture, no strikethrough. No moisture, no strikethrough. I'll now look inside to check for the chemical indicator to make sure it went through the sterile process. Show my circulator and hand off. I will also hand off the count sheet. And now my circulator will check the bottom to make sure that it is still sterile. Sterile.
CHAPTER 6
Next, I will take all the instruments and retractors out to set them up accordingly. I'll place my instruments on my pre-rolled towel. I will now check the bottom of the tray to see if there are any instruments or moisture. I will now place my forceps and my retractors inside of my tray as follows. I will separate my forceps from atraumatic and traumatic. I will place my knife handles on my back table's neutral zone. As well as the smooth grooved director.
CHAPTER 7
Now that everything is separated and organized, I will begin the initial count with my circulator. Starting with laps, I have one, two, three, four, five. My Ray-Tecs, I have one, two, three, four, five, six, seven, eight, nine, 10. Blades, I have one, two, three. Sutures, I have one, two, three. Hypodermic needles, I have one. Bovie, I have one with one tip and one scratch pad. And now onto my stringer. Mosquitoes, I have one, two, three, four. Criles, I have one, two, three, four, five, six, seven, eight. Kellys, I have one, two, three, four, five, six, seven, eight. Right angles, I have one, two, three, four. Allises, I have one, two. Kochers, I have one, two. Tonsils, I have one, two. Babcocks, I have one, two. Needle holders, I have one, two, three, four. Metzenbaum scissors, I have one, two. Curved Mayo scissors, I have one. Straight Mayo scissors, I have one. Sponge sticks, I have one, two, three, four. On to my forceps. Starting with the traumatic ones, I have rat tooths, one, two. I have Adsons, one, two. My atraumatic ones, I have plain tissue forceps, one, two. And DeBakeys one, two. On to my retractors. Richardsons, I have one, two, three, four. Army-Navys, I have one, two. Farabeufs, I have one, two. Volkmann Rakes, I have one. Senn retractors, I have one, two. And Weitlaners, I have one, two. Knife handles, I have one, two, three, four, five. And I have one Yankauer suction. Based upon my facility, we count these as two pieces because it has one tip. And I have one smooth grooved director. I will verify with my circulator that the initial count is complete. Count is complete? Initial count, complete. Initial count is complete.
CHAPTER 8
Next, I will remove my stringer and get my instruments onto my Mayo stand to prep for the surgery. I will start by loading my blades and putting them onto my Mayo stand. I place them onto my neutral zone on my Mayo stand. Next, I will add my syringe to the neutral zone. I will bring some lap sponges. Next, I will add the marking pen. Now I will begin adding instruments, starting with Criles for hemostasis. I will next add two Allises to aid in atraumatic tissue manipulation. A straight Mayo scissors for the sutures. Metzenbaum scissors for dissection. I will also add tonsils for blunt tissue dissection depending on the surgeon's preference. I will add Senn retractors, as well as Richardson retractors depending on the size and the surgeon's preference. Once my setup is complete, we are ready to gown and glove the surgical team members, and we are now ready for the patient to come in.
